Question style: what is and is not official
The current official pages say only that the exam uses MCQs. They do not state:
- whether every item is single best answer;
- how many answer options each question has;
- whether there is negative marking;
- the exam language (English is strongly implied by the official documents, sample questions and reading list, but there is no explicit statement).
The only official sample is a legacy 2012 document titled “EMSTREX 2012” on the exam board site. It uses clinical-vignette, single-best-answer questions with four options. Some commercial sites report four to five options and no negative marking based on candidate feedback; this is not confirmed officially. Our practice questions use four-option single-best-answer vignettes, matching the official sample.
How the 120 questions are distributed
The blueprint weights seven disciplines. The question counts in the right-hand column are our arithmetic (percentage × 120); the blueprint publishes percentages only, and distributions may differ by up to ±5% in each category.
| Discipline | Official weight | Approx. questions (our calculation) |
|---|---|---|
| Internal Medicine | 20% | 24 |
| Obstetrics and Gynaecology | 15% | 18 |
| Paediatrics | 15% | 18 |
| Surgery | 15% | 18 |
| Family Medicine | 15% | 18 |
| Psychiatry | 10% | 12 |
| Public Health / Preventive Medicine | 10% | 12 |
The official difficulty split is 40% easy (48 questions), 40% moderately difficult (48) and 20% difficult (24). Competency weightings are covered on the blueprint page.
Pilot questions and item review
The FAQ explains that the board includes “some test questions which are not intended for scoring”, so you may see more than 120 items; extra time is given to answer them. You will not know which items are pilots, so treat every question seriously.
After each sitting, all items undergo psychometric analysis and “poorly performing questions are removed before recalculating the final result”. Your percentage is therefore based on the scored items only.
Because pilot items come with extra time, the average pace per scored question stays similar. Do not spend long trying to guess which items are unscored; answer every question as if it counts.
Exam-day rules
The official rules page sets out the following:
- Face recognition and an official ID document (passport and/or UAE ID card) are a must.
- A 15-minute practice session is available, if required, to get used to the computer-based system.
- Lockers are provided for personal belongings.
- Candidates arriving more than 10 minutes late are not allowed to enter.
- No one may leave the exam room during the first 30 minutes.
- There is one invigilator per 20 candidates.
- The regulations of the NIHS Assessment and Evaluation Center apply.
No Pearson VUE or Prometric delivery, and no overseas test centre, is mentioned on any official page. Candidates from abroad need their own UAE visa to attend.
How to train for the pattern (our recommendations)
- Pace: practise at about 1.5 minutes per question. Our timed practice tests use this pace by default.
- Stamina: sit at least two or three 120-question, 3-hour mocks before exam day.
- Clinical reasoning: because the focus is diagnosis and management, practise reading vignettes for the key finding and next best step, not isolated facts.
- Balance: spread your study by blueprint weight using the subject banks.
